CHA Recertification Documents

The following forms are for tenants who are currently active on CHA programs or who have been issued an Application Checklist

You will be responsible for obtaining and providing any documentation pertaining to your household regarding SNAP benefits, Veteran benefits, Unemployment, Social Security, Workman's Comp, Financial Aid, class schedules, retirement or pensions received. You are responsible for providing all documentation pertaining to your household even if the item is not checked on your recertification checklist.
